Researchers from King’s, with doctors at King’s College Hospital NHS Foundation Trust, have successfully used a new robot system to improve treatment for debilitating eye disease.

The custom-built robot was used to treat wet neovascular age-related macular degeneration (AMD), administering a one-off, minimally invasive dose of radiation, followed by patients’ routine treatment with injections into their eye.

In the landmark trial, published in The Lancet, it was found that patients then needed fewer injections to effectively control the disease, potentially saving around 1.8 million injections per year around the world.

Wet AMD is a debilitating eye disease, where abnormal new blood vessels grow into the macula, the light sensing-layer of cells inside the back of the eyeball. The vessels then start to leak blood and fluid, typically causing a rapid, permanent and severe loss of sight.

Globally, around 196 million people have AMD and the Royal College of Ophthalmologists estimates that the disease affects more than 700,000 people in the UK. The number of people with AMD is expected to increase 60% by 2035, due to the country’s ageing population.

Wet AMD is currently treated with regular injections into the eye. Initially, treatment substantially improves a patient’s vision. But, because the injections don’t cure the disease, fluid will eventually start to build up again in the macula, and patients will require long-term, repeated injections. Most people require an injection around every 1-3 months, and eye injections, costing between £500 and £800 per injection, have become one of the most common NHS procedures.

The new treatment can be targeted far better than existing methods, aiming three beams of highly focused radiation into the diseased eye. Scientists found that patients having robotic radiotherapy required fewer injections to control their disease compared to standard treatment.

The study found that the robotically controlled device saves the NHS £565 for each patient treated over the first two years, as it results in fewer injections.

The study lead and first author on the paper, Professor Timothy Jackson, King’s College London and Consultant Ophthalmic Surgeon at King’s College Hospital said: “Research has previously tried to find a better way to target radiotherapy to the macula, such as by repurposing devices used to treat brain tumours. But so far nothing has been sufficiently precise to target macular disease that may be less than 1 mm across.

“With this purpose-built robotic system, we can be incredibly precise, using overlapping beams of radiation to treat a very small lesion in the back of the eye.

“Patients generally accept that they need to have eye injections to help preserve their vision, but frequent hospital attendance and repeated eye injections isn’t something they enjoy. By better stabilising the disease and reducing its activity, the new treatment could reduce the number of injections people need by about a quarter. Hopefully, this discovery will reduce the burden of treatment that patients have to endure.”

Dr Helen Dakin, University Research Lecturer at the University of Oxford said: “We found that the savings from giving fewer injections are larger than the cost of robot-controlled radiotherapy. This new treatment can therefore save the NHS money that can be used to treat other patients, while controlling patients’ AMD just as well as standard care.”

The research was jointly funded by the National Institute for Health and Care Research (NIHR) and the Medical Research Council (MRC) and recruited 411 participants across 30 NHS hospitals. A Lancet-commissioned commentary that accompanied the article described it as a “landmark trial.”

This study was led by researchers from King’s College London and doctors at King’s College Hospital NHS Foundation Trust, in collaboration with the University of Oxford, the University of Bristol and Queen’s University in Belfast.

Journal Reference:

Timothy L Jackson, Riti Desai, Hatem A Wafa, Yanzhong Wang, Janet Peacock, Tunde Peto, Usha Chakravarthy, Helen Dakin, Sarah Wordsworth, Cornelius Lewis, Patricia Clinch, Lisa Ramazzotto, James E Neffendorf, Chan Ning Lee, Joe M O’Sullivan, Barnaby C Reeves, Salwa Abugreen, Mandeep Bindra, Ben Burton, Indra Dias, Christiana Dinah, Ravikiran Gandhewar, Athanasios Georgas, Srinivas Goverdhan, Ansari Gulrez, Richard Haynes, Edward Hughes, Timothy Jackson (Chief Investigator), Afsar Jafree, Sobha Joseph, Tarek Kashab, Luke Membrey, Geeta Menon, Aseema Misra, Niro Narendran, Douglas Newman, Jignesh Patel, Sudeshna Patra, Robert Petrarca, Prakash Priya, Arora Rashi, Ramiro Salom, Paritosh Shah, Izadi Shahrnaz, George Sheen, Marianne Shiew, Paul Tesha, Eleni Vrizidou. Stereotactic radiotherapy for neovascular age-related macular degeneration (STAR): a pivotal, randomised, double-masked, sham-controlled device trial. The Lancet, 2024; DOI: 10.1016/S0140-6736(24)00687-1

An artificial intelligence (AI) system that can recommend the correct referral decision for over 50 eye diseases as accurately as world-leading experts has been developed by researchers at Moorfields Eye Hospital NHS Foundation Trust, DeepMind Health and University College London (UCL) Institute of Ophthalmology.

The breakthrough research, published online by Nature Medicine, describes how machine learning technology has been successfully trained on thousands of historic de-personalised eye scans to identify features of eye disease and recommend how patients should be referred for care. It is hoped that the technology could one day transform the way professionals carry out eye tests, allowing them to spot conditions earlier and prioritise patients with the most serious eye diseases before irreversible damage sets in.

More than 285 million people worldwide live with some form of sight loss, including more than two million people in the UK. Eye diseases remain one of the biggest causes of sight loss, and many can be prevented with early detection and treatment.

Dr Pearse Keane, consultant ophthalmologist at Moorfields Eye Hospital NHS Foundation Trust and NIHR Clinician Scientist at the UCL Institute of Ophthalmology said:

“The number of eye scans we’re performing is growing at a pace much faster than human experts are able to interpret them. There is a risk that this may cause delays in the diagnosis and treatment of sight-threatening diseases, which can be devastating for patients.

“The AI technology we’re developing is designed to prioritise patients who need to be seen and treated urgently by a doctor or eye care professional. If we can diagnose and treat eye conditions early, it gives us the best chance of saving people’s sight. With further research it could lead to greater consistency and quality of care for patients with eye problems in the future.”

The study, which was launched in 2016, brought together leading NHS eye health professionals and scientists from the National Institute for Health Research (NIHR) and UCL with some of the UK’s top technologists at DeepMind to investigate whether AI technology could help improve the care of patients with sight-threatening diseases, such as age-related macular degeneration and diabetic eye disease.

Using two types of neural network – mathematical systems for identifying patterns in images or data – the AI system quickly learnt to identify ten features of eye disease from highly complex optical coherence tomography (OCT) scans. The system was then able to recommend a referral decision based on the most urgent conditions detected.

To establish whether the AI system was making correct referrals, clinicians also viewed the same OCT scans and made their own referral decisions. The study concluded that AI was able to make the right referral recommendation more than 94% of the time, matching the performance of expert clinicians.

The AI has been developed with two unique features which maximise its potential use in eye care. Firstly, the system can provide information that helps explain to eye care professionals how it arrives at its recommendations. This information includes visuals of the features of eye disease it has identified on the OCT scan and the level of confidence the system has in its recommendations, in the form of a percentage. This functionality is crucial in helping clinicians scrutinise the technology’s recommendations and check its accuracy before deciding the type of care and treatment a patient receives.

Secondly, the AI system can be easily applied to different types of eye scanner, not just the specific model on which it was trained. This could significantly increase the number of people who benefit from this technology and future-proof it, so it can still be used even as OCT scanners are upgraded or replaced over time.

The next step is for the research to go through clinical trials to explore how this technology might improve patient care in practice, and regulatory approval before it can be used in hospitals and other clinical settings.

If clinical trials are successful in demonstrating that the technology can be used safely and effectively, Moorfields will be able to use an eventual, regulatory-approved product for free across all 30 of their UK hospitals and community clinics, for an initial period of five years.

The work which has gone into this project will also help accelerate wider NHS research for many years to come. For example, DeepMind has invested significant resources to clean, curate and label Moorfields’ de-identified research dataset to create one of the most advanced eye research databases in the world.

Moorfields owns this database as a non-commercial public asset, which is already forming the basis of nine separate medical research studies. In addition, Moorfields can also use DeepMind’s trained AI model for future non-commercial research efforts, which could help advance medical research even further.

Mustafa Suleyman, Co-founder and Head of Applied AI at DeepMind Health, said:
“We set up DeepMind Health because we believe artificial intelligence can help solve some of society’s biggest health challenges, like avoidable sight loss, which affects millions of people across the globe. These incredibly exciting results take us one step closer to that goal and could, in time, transform the diagnosis, treatment and management of patients with sight threatening eye conditions, not just at Moorfields, but around the world.

“We’re immensely proud of this work, which once again demonstrates what is possible when world-leading clinicians and technologists collaborate to improve patient care”.

Professor Sir Peng Tee Khaw, director of the NIHR Biomedical Research Centre at Moorfields Eye Hospital NHS Foundation Trust and UCL Institute of Ophthalmology said:
“The results of this pioneering research with DeepMind are very exciting and demonstrate the potential sight-saving impact AI could have for patients. I am in no doubt that AI has a vital role to play in the future of healthcare, particularly when it comes to training and helping medical professionals so that patients benefit from vital treatment earlier than might previously have been possible. This shows the transformative research than can be carried out in the UK combining world leading industry and NIHR/NHS hospital/university partnerships.”

Elaine Manna, 71, from north London and mother of three, went blind in her left eye from wet age-related macular degeneration (AMD). She is now being treated at Moorfields Eye Hospital to save the remaining sight in her right eye. She said:
“I lost the sight in my left eye in the year 2000 after noticing a sudden blurring of my vision. A few weeks went by and I was starting to get really worried. A doctor then told me I had a bleed at the back of my eye that needed to be treated urgently but it proved too late to save my sight.

“AMD has had a major impact on my life. I love going to the theatre but I find it difficult to see people’s faces and I struggle to find my way around dimly lit rooms. I really hope that technology can help people like me in the future.”

Matt Hancock, Health and Social Care Secretary, said:
“The UK leads the world in ground-breaking research and artificial intelligence has the potential to revolutionise the way we spot eye disease, and could be genuinely transformative for the NHS in the future.

“This is hugely exciting and exactly the type of technology which will benefit the NHS in the long term and improve patient care – that’s why we fund over a billion pounds a year in health research as part of our long term plan for the NHS.”

Robert Dufton, chief executive at Moorfields Eye Charity, said:
“The need for treatment for eye diseases is forecast to grow, in part because people are living longer, far beyond our ability to meet the demand using current practice. Artificial intelligence is showing the potential to transform the speed at which diseases can be diagnosed and treatments suggested, making the best use of the limited time of clinicians. AI will also help our understanding of sight loss. Moorfields Eye Charity is proud to have funded equipment which underpins Dr Pearse Keane’s work as part of our programme of philanthropic support in pioneering researchers.”

Tony Young, NHS England’s National Clinical Director for Innovation, said:
“The NHS has always nurtured medical breakthroughs and now artificial intelligence, although still in its infancy, has real potential to radically transform the way medicine is practiced. This exciting research provides important evidence about how AI can support doctors to provide more rapid diagnosis and treatment, freeing up clinicians’ time for direct patient care.”

Cathy Yelf, chief executive of the Macular Society, said:
“Macular disease is the biggest cause of sight loss in the UK, affecting 600,000 people. It is extremely debilitating and timely treatment for patients with wet AMD, such as Elaine, is vital. Pressure on eye clinics has resulted in delays for many patients, which has tragically led to unnecessary sight loss. We’re excited by this latest development and hope in time this technology will alleviate the pressure on clinics and mean patients will get the urgent treatment they need.”

Martin Cordiner, Head of Research at the College of Optometrists said:
“As optometrists are often the first port of call for people with the symptoms of eye disease, we are very excited about the potential that AI has to assist them in helping patients. Further support in prioritising the referral of the patients with the greatest need is in the interests of both practitioners and patients, and we look forward to the results of clinical trials of this technology.”

Michele Acton, Chief Executive at Fight for Sight, said:
“Artificial intelligence has the potential to help us address so many challenging health issues. We’re delighted that it is being used to help those patients most at need to receive timely access to ophthalmologists and appropriate treatments. This is a great step forward and we believe that the use of artificial intelligence will have an increasing role in addressing eye diseases in the future.”

Professor Andrew Lotery, Chair Scientific Committee, The Royal College of Ophthalmologists, said:
“This paper shows the power of utilizing artificial intelligence in ophthalmology. Innovative research such as this could help hospital eye services manage their clinics more effectively in the future.”

Originally published in World Association of Eye Hospitals.

There is absolutely not a single doubt that those patterns of marketing strategies have yielded tremendous results in Optometry. In fact, those are marketing practices that have been developed by practice owners who are demonstrating excellent entrepreneurial behavior time after time and are remarkably paving the way to other practitioners to employ and improve those strategies. In this article, I try to delve first into listing what is commercial marketing that allows myopia management practitioners to market their practices and then define social marketing strategies for myopia management trying to demonstrate how commercial marketing fits into social marketing the myopia disease.

Social media and other content-sharing platforms offer countless chances for online interaction these days. Our daily lives are largely reliant on the internet, which provides the best means of obtaining news and information. As a matter of fact, eight out of ten individuals state that they get most of their news from digital devices, such as social media apps or online news sources (Shearer, 2021). Health experts and eye care professionals in particular can use social marketing and other health promotion methods at different levels in their marketing strategy.

Social marketing, on the other side, employs marketing techniques typically used in commerce to influence behavior regarding social issues. Crafting a social marketing strategy enables a more targeted and effective approach in communicating with specific audiences, leading to improved outcomes. It’s essential to differentiate social marketing from social media, as the latter, including platforms like Facebook and Twitter, serves as tools within the broader framework of social marketing plans to achieve specific objectives.

Optometry practices are implementing different commercial marketing strategies and social media strategies to effectively raise awareness, attract patients, and establish themselves as a trusted provider in the community. Here are a few of them:

Patient Education during Exams: Utilize exam time to educate patients and parents about myopia management, emphasizing the risks associated with untreated myopia and the benefits of early intervention (Kazmi, 2022).

E-Blast Marketing: Target e-blasts to the patient base, highlighting new treatment options or services. Segment lists to focus on patients who may benefit from myopia management (Kazmi, 2022).

Blog & Vlog Series: Establish a regular blog and vlog series to provide in-depth information about myopia, its prevalence, associated risks, and available treatment options. Consistent content creation builds credibility and engages the community (Kazmi, 2022).

Staff Training and Education: Educate office staff about myopia management services and treatments. Provide materials and resources to facilitate patient education and engagement. Ensure staff members can effectively communicate the benefits of myopia management (Aleman-Moheeputh,2021).

Word-of-Mouth Marketing: Encourage satisfied patients to spread the word about the practice’s myopia management services. Leverage patient testimonials and success stories to build trust and credibility (Aleman-Moheeputh,2021).

Networking with Local Optometrists: Foster collaborative relationships with other eye care professionals to exchange referrals and support. Participate in local events and professional organizations to expand the practice’s network (Aleman-Moheeputh,2021).

Online Presence and Social Media: Maintain an updated Google business listing and actively engage on social media platforms. Share informative content, success stories, and practice updates to stay top-of-mind among patients and colleagues (Aleman-Moheeputh,2021).

Andreasen (1995), in the preface to “Marketing Social Change,” defines social marketing as applying commercial strategies to address various socially significant behaviors, like drug use and smoking. This approach, drawing from established concepts and techniques, has substantial potential to address significant societal issues if effectively utilized. The “proven techniques” encompass methods from behavioral theory, persuasion psychology, and marketing science. These techniques involve leveraging behavioral theory to influence health-related behaviors, evaluating audience receptivity to messages, and strategically marketing messages to drive behavior change using the “four Ps” of marketing (Kotler & Lee, 2008):

1- Behavior Change: Initiate campaigns targeting behavior change towards myopia prevention, emphasizing the importance of outdoor activities and reducing screen time for eye health.

2- Customer Research: Conduct comprehensive research to understand myopia prevalence, risk factors, and prevailing attitudes towards eye health within target populations, enabling tailored intervention strategies.

3- Audience Segmentation: By segmenting their target audience based on factors such as age, lifestyle, and eye health needs, optometrists can tailor their marketing messages to resonate with specific groups. For example, they can develop messages targeted at parents concerned about their children’s eye, the need for early eye exam, emmetropes prevention ways, pre-myopic management tools, myopia, and high myopia management methods.

4- Marketing Mix:

· Product: Position outdoor activities as desirable ‘products’ associated with myopia prevention, offering benefits such as improved overall health and reduced risk of vision impairment.

· Price: Address perceived barriers such as inconvenience or lack of awareness by offering incentives like free outdoor activity guides or discounts on protective eyewear.

· Place: Promote myopia prevention interventions in settings where the target audience spends significant time, such as schools, workplaces, and online platforms.

· Promotion: Utilize various channels including social media, educational campaigns, and partnerships with influencers to amplify messages and engage the audience effectively.

5- Communication Channels: Optometrists can utilize a variety of communication channels to reach their target audience, including social media, community events, and local advertising. By engaging with potential patients through these channels, they can raise awareness about the importance of regular eye exams and promote their services effectively.

6- Tailored Messages: Optometrists can create personalized messages that address the unique needs and concerns of their target audience. For example, they can highlight the benefits of specific eye care services such as myopia management at the juvenile onset.

7- Brand Building: By developing a strong brand identity that emphasizes expertise, trustworthiness, and patient-centered care, optometrists can differentiate themselves from competitors and attract more patients. Consistent branding across all marketing channels helps reinforce their reputation and build long-term relationships with patients.

8- Reinforcement of Public Health Messages: Optometrists can reinforce public health messages related to eye care, such as the importance of regular screenings and good visual habits like less screen time. By aligning their marketing efforts with broader public health initiatives, they contribute to raising awareness and promoting positive behavior change in the community.

9- Community Engagement: Optometrists can actively engage with their local community by participating in health fairs, school events, and other outreach activities. These opportunities not only allow them to educate the public about eye health but also establish themselves as trusted resources within the community.

Employing social marketing techniques in myopia management can be beneficial. Initially, within initiatives like encouraging time spent outdoor and less screen time campaigns, eye care professionals should augment media messages with brief counseling. Additionally, they can enhance outreach by offering an additional avenue for communication to engage the target audience. Lastly, leveraging their credibility as trusted sources of health information, practitioners amplify the impact of social marketing messages, extending their influence beyond conventional commercial marketing and mass communication methods.

References:

Aleman-Moheeputh, G. How To Market Myopia Management in a Retail Practice. 2021. This amounts to 1 per cent of all non-fatal occupational injuries.

Eye health significantly affects labour markets: workers with vision impairment are 30 per cent less likely to be employed, compared to those without. Economic development plays a significant role in the prevalence of vision impairment, with low- and middle-income regions experiencing about four times more cases than high-income regions.

More than 90 per cent of vision impairment cases are either preventable or treatable through existing, highly cost-effective interventions, emphasizes the study. This underscores the need for coordinated global, national, and workplace initiatives to protect workers’ well-being, since eye health is an integral part of workers’ health.

Occupational safety and health (OSH) programmes to protect the vision of workers should be designed with three goals in mind, underlines the study: prevent exposure to specific hazards in each workplace; protect the existing health of workers’ eyes; and provide a system to include workers’ naturally occurring sight loss in risk assessments.

Workers should be informed of hazards that may affect their eye health. Workers and their representatives should also be consulted about eye health programmes and interventions in workplaces.

“The ILO stresses the importance of protecting workers’ safety and health – including their eyes. By prioritizing eye health awareness and effective implementation, we can make sure workers have access to a safe and healthy working environment. This ensures their overall well-being, reduces disparities, and leads to heightened productivity,” said Joaquim Pintado Nunes, ILO Chief of Labour Administration, Labour Inspection and Occupational Safety and Health.

“This report demonstrates to everyone the vital importance of looking after our eyes, whilst also giving incredibly useful guidance and recommendations for how to protect and promote eye health within the workplace,” said IAPB President, Caroline Casey.

Asia Pacific Academy of Ophthalmology is one of the largest gathering of ophthalmologists (and associated members of the eye health sector) every year. In Bali, for 2024, the congress provided the perfect opportunity to bring together IAPB members and stakeholders from several WHO regions including the Western Pacific, Southeast Asia, Eastern Mediterranean and North America. Beachside at Nusa Dua, this year’s event combined networking with the soft-launch of the Joint IAPB/IDF Policy Brief on Diabetic Retinopathy.

The brief will be formally released in an online event at the end of March, however, Indonesia is a fitting location for a sneak peak – the country is now within the top 10 countries for prevalence, globally. This was acknowledged by Dr Eva Susanto, representing Indonesia’s Ministry of Health and who assured attendees of the work being done by the govt to address the increasing burden. Co-author, Professor Serge Resnikoff, urged IAPB members to see the brief as an essential advocacy tool and opportunity to speak to governments with one voice.

This is the power of IAPB: a diverse group of members and stakeholders, who can uniquely speak with one voice. #strongertogether. Special thanks to I-care international for supporting the event and allowing us to come together, to learn and network, to make new friends and renew old acquaintances.

Barcelona, Spain – In a new phase of the journey, which began in Morocco and later visited Marseille, Sea2see’s creative team visited the fish markets of Seoul and their vibrant fish sellers’ stalls. The photographer and filmmaker, Edouard Valette, worked again with local models in South Korea, creating the memorable visuals which feature Sea2see products.

The image series has been fully integrated as a key focus of the Sea2see visual campaign for 2024, acting as a reminder that in all parts of the world, the problems of plastic contamination remains critical, even if unseen in daily life or in places such as local fish markets which are completely reliant on the bounty of the sea.

“I have integrated our photographic series by Edouard Valette into our campaign for 2024 to provoke thought, action, commitment to our cause. Behind Sea2see eyewear, we are always focused on raising further awareness about ocean pollution and collecting and removing polluting plastics from the environment. These images, even if fashion, are a poignant reminder of what we must do,” said François van den Abeele, Founder of Sea2see.

All Sea2see frames are produced entirely in Italy with upcycled marine plastic. Each frame will finance the collection of 1kg of marine plastic.

A Johns Hopkins Children’s Center study of children and youth with diabetes concludes that so-called autonomous artificial intelligence (AI) diabetic eye exams significantly increase completion rates of screenings designed to prevent potentially blinding diabetes eye diseases (DED). During the exam, pictures are taken of the backs of the eyes without the need to dilate them, and AI is used to provide an immediate result.

The study noted that the AI-driven technology used in the exams may close “care gaps” among racial and ethnic minority youth with diabetes, populations with historically higher rates of DED and less access to or adherence with regular screening for eye damage.

In a report on the study published Jan. 11 in Nature Communications, investigators examined diabetic eye exam completion rates in people under age 21 with type 1 and type 2 diabetes, and found that 100% of patients who underwent AI exams completed the eye assessment.

DED primarily refers to diabetic retinopathy, a potentially blinding complication of diabetes that occurs when poorly controlled sugar levels cause the overgrowth of, or damage to, blood vessels and nerve tissues in the light-sensitive retina at the back of the eye. According to the study researchers, retinopathy affects between 4% and 9% of youth with type 1 diabetes, and 4% to 15% of youth with type 2 diabetes. About 238,000 children, adolescents and young adults under age 20 are estimated to have diagnosed diabetes, according to the American Diabetes Association. Frequent screenings for DED facilitate early detection and treatment, and can help prevent progression of DED.

Generally, diabetes specialists and eye doctors recommend annual screenings, which typically require an additional, separate visit to an eye care provider, such as an optometrist or ophthalmologist, and the use of drops to dilate the pupil so that a clear view of the retina is visible through specialized instruments. However, studies show only 35% to 72% of youth with diabetes undergo recommended screenings, with even higher care gap rates among minority and poor youth. Previous studies also show that barriers to screenings include confusion about the need for screenings, inconvenience, and lack of time, access to specialists and transportation.

Previous studies by Risa Wolf, M.D., a pediatric endocrinologist at Johns Hopkins Children’s Center, and her team have found autonomous AI screening that uses cameras produce results that enable accurate DED diagnosis.

In the new study, researchers enrolled 164 participants, ranging in age from 8 to 21 years and all from the Johns Hopkins Pediatric Diabetes Center, between Nov. 24, 2021, and June 6, 2022. Some 58% were female and 41% were from minority groups (35% Black; 6% Hispanic). Some 47% of participants had Medicaid insurance. The subjects were randomly assigned to one of two groups. A group of 83 patients received the standard screening instructions and care, and were referred to either an optometrist or ophthalmologist for an eye exam. A second group of 81 patients underwent a five-to-10-minute autonomous AI system diabetic eye exam during a visit to their endocrinologist (the specialists who typically care for people with diabetes), and received their results at the same visit.

The AI system takes four pictures of the eye without dilation, and runs the images through an algorithm that determines the presence or absence of diabetic retinopathy, Wolf says. If it is present, a referral is made to an eye doctor for further evaluation. If it is absent, “you’re good for the year, and you just saved yourself time,” she adds.

Researchers found that 100% of patients in the group offered the autonomous AI screening completed their eye exam that day, while 22% of patients from the second group followed through within six months to complete an eye exam with an optometrist or ophthalmologist. The researchers found no statistical differences based on race, gender or socioeconomic status for whether participants in the second group scheduled the separate screening with an eye doctor.

The researchers also found that 25 out of 81 participants, or 31%, in the autonomous AI group had a result indicating that DED was present. Sixteen of those participants, or 64%, followed through in scheduling a secondary appointment with an eye care provider. Further analysis showed those who did not schedule the appointment were more likely to be Black and have Medicaid insurance.

“With AI technology, more people can get screened, which could then help identify more people who need follow-up evaluation,” says Wolf. “If we can offer this more conveniently at the point of care with their diabetes doctor, then we can also potentially improve health equity, and prevent the progression of diabetic eye disease.”

The investigators caution that the autonomous AI used in their study is not approved by the U.S. Food and Drug Administration for those under 21 years old. And they say a potential source of bias in the study was that some of the participants were familiar with autonomous AI diabetic eye exams from a prior study, and therefore may have been more willing to participate in the new one.

Along with Wolf, the study authors from Johns Hopkins include Alvin Liu, Anum Zehra, Lee Bromberger, Dhruva Patel, Ajaykarthik Ananthakrishnan, Elizabeth Brown, Laura Prichett and Harold Lehmann. Other authors are Roomasa Channa from University of Wisconsin and Michael D. Abramoff from the University of Iowa.

The study was funded by the National Eye Institute of the National Institutes of Health (Award Number R01EY033233) and the Diabetes Research Connection.

The authors associated with The Johns Hopkins University declared no conflicts of interest related to this press release.

Journal Reference:

Risa M.
In a study recently published in JAMA Ophthalmology, researchers from the Tokyo Medical and Dental University (TMDU) developed a machine-learning model that works well for predicting — and visualizing — the risk of visual impairment over the long term.

People with extreme shortsightedness (called high myopia) can clearly see objects that are near to them but cannot focus on objects at a distance.

Contacts, glasses, or surgery can be used to correct their vision, but having high myopia is not just inconvenient; half of the time it leads to a condition called pathologic myopia, and complications from pathologic myopia are the leading causes of blindness.

“We know that machine-learning algorithms work well on tasks such as identifying changes and complications in myopia,” says Yining Wang, lead author of the study, “but in this study, we wanted to investigate something different, namely how good these algorithms are at long-term predictions.”

To do this, the team performed a cohort study and looked at the visual acuity of 967 Japanese patients at TDMU’s Advanced Clinical Center for Myopia after 3 and 5 years had passed.

They formed a dataset from 34 variables that are commonly collected during ophthalmic examinations, such as age, current visual acuity, and the diameter of the cornea.

They then tested several popular machine-learning models such as random forests and support vector machines.

Of these models, the logistic regression-based model performed the best at predicting visual impairment at 5 years.

However, predicting outcomes is only part of the story. “It’s also important to present the model’s output in a way that is easy for patients to understand and convenient for making clinical decisions,” says Kyoko Ohno-Matsui, senior author.

To do this, the researchers used a nomogram to visualize the classification model.

Each variable is assigned a line with a length that indicates how important it is for predicting visual acuity.

These lengths can be converted into points that can be added up to obtain a final score explaining the risk of visual impairment in future.

People who permanently lose their vision often suffer both financially and physically as a result of their loss of independence. The decrease in global productivity caused by severe visual impairment was estimated to be USD94.5 billion in 2019. Although the model still has to be evaluated on a wider population, this study has shown that machine-learning models have good potential to help address this increasingly important public health concern, which will benefit both individuals and society as a whole.

Journal Reference:

Yining Wang, Ran Du, Shiqi Xie, Changyu Chen, Hongshuang Lu, Jianping Xiong, Daniel S.
Padua – Safilo Group, one of the eyewear industry’s key players in the design, manufacturing, and distribution of prescription frames, sunglasses, outdoor eyewear, goggles, and helmets, has signed an agreement for the acquisition of a minority stake in Spaarkly, a digitally native company that specializes in developing innovative e-commerce solutions using augmented reality technologies.

The collaboration with Spaarkly began in December 2022 when Safilo launched Polaroid’s new “Instant Try-on” experience, allowing everyone to virtually and instantly try the new Polaroid collection. By simply scanning a QR code with a smartphone, users are offered a fast, immersive, and enjoyable purchasing experience that links virtual and physical media touchpoints.

Since then, Safilo has been using Spaarkly’s ARshades, a complete suite of software and services for virtual eyewear try-on, to enhance above all Polaroid and Carrera’s ecommerce websites.

“We are proud to strengthen our partnership with Sparkly, an innovative start-up specialized in augmented reality and the development of solutions for e-commerce. This partnership aims to leverage Spaarkly’s capabilities, bringing to the market increasingly advanced AR solutions and other disruptive, cutting edge products. In line with our digital transformation strategy we want to keep looking for “sparkling” solutions, which can enhance and support the development of our DTC and BTB channels, offering final consumers and clients alike new shopping experiences and selling opportunities.” Declared Angelo Trocchia, Safilo Group CEO.
