Better still, it’s neither difficult nor expensive. In fact, doing it well may actually save you time, boosting your profits, and help prevent vision loss.</span></p><p><span style="font-weight: 400; font-family: Thabit, sans-serif;">Here’s why focusing our efforts on diabetes management is a worthwhile endeavor for us and for our patients.</span></p><h4><span style="font-family: Thabit, sans-serif;"><b>What We Can Do For Our Patients</b></span></h4><p><span style="font-weight: 400; font-family: Thabit, sans-serif;">There are several excellent, altruistic reasons to focus more of our attention on diabetic eyecare:</span></p><ol><li style="font-weight: 400;" aria-level="1"><span style="font-family: Thabit, sans-serif;"><b><i>We can help tackle a growing epidemic</i></b><i><span style="font-weight: 400;">. </span></i><span style="font-weight: 400;">Diabetes has reached epidemic proportions globally, and its impact on eye health cannot be overstated. By concentrating on diabetes care, optometrists position themselves as front-line guardians against a pervasive health concern. With the prevalence of diabetes rising, the demand for specialized eyecare is more critical than ever. Optometry practices that focus on diabetes care contribute positively to public health outcomes. Actively managing diabetic eye disease by monitoring both the structure and function of the visual system enables us to play a crucial role in preventing vision loss and improving overall patient well-being. This commitment to public health can enhance our practice’s reputation and community standing.</span></span></li><li style="font-weight: 400;" aria-level="1"><span style="font-family: Thabit, sans-serif;"><b><i>We can help address health disparities</i></b><i><span style="font-weight: 400;">.</span></i><span style="font-weight: 400;"> Health equity is everyone’s responsibility, and the need for diabetic eyecare services is pronounced. For example, the Latino/Hispanic population is more likely to develop diabetes-related complications, often has a higher mortality rate and receives lower-quality healthcare compared to mainstream groups.</span><span style="font-weight: 400;">1 </span><span style="font-weight: 400;">Geography likewise contributes to inequities in diabetes-related care. A recent analysis of more than 28 million eye exams performed by U.S. optometrists and ophthalmologists showed a significant geographic gap in care and provider coverage in counties with lower median household incomes, higher poverty rates and fewer high school graduates.</span><span style="font-weight: 400;">2</span></span></li><li style="font-weight: 400;" aria-level="1"><span style="font-family: Thabit, sans-serif;"><b><i>We can elevate standards of care.</i></b> <span style="font-weight: 400;">In 2023, I joined several colleagues in authoring a consensus document on the assessment and management of diabetic retinopathy management in optometry.</span><span style="font-weight: 400;">3 </span><span style="font-weight: 400;">Me and the other members of the task force have a singular purpose: to elevate the standard of care to avoid preventable vision loss. We developed a framework structured around five guidelines that clarify best practices in detection, grading, risk assessment, management and patient support. More recently, we issued a call for comment from the optometric community to help ensure that the guidance represents the voice of our profession. </span></span></li></ol><h4><span style="font-family: Thabit, sans-serif;"><b>What We Can Do For Our Practices</b></span></h4><ol><li style="font-weight: 400;" aria-level="1"><span style="font-family: Thabit, sans-serif;"><b><i>We can reap the rewards of supply and demand.</i></b><span style="font-weight: 400;"> Diabetes care can be financially rewarding for optometry practices. With a growing population of people with diabetes, there is a consistent demand for specialized eyecare services. Moreover, the establishment of long-term patient relationships ensures a steady stream of revenue, contributing to the financial viability and sustainability of our practices. Income generation stems from diagnostic testing, such as OCT and ERG, as well as from nutritional supplement dispensing when appropriate from a medical and intervention standpoint.</span></span></li><li style="font-weight: 400;" aria-level="1"><span style="font-family: Thabit, sans-serif;"><b><i>We can differentiate ourselves in a competitive landscape.</i></b><span style="font-weight: 400;"> Focusing on diabetes care allows optometrists to differentiate their practices. The ability to provide comprehensive eye care tailored specifically to diabetic patients can set a practice apart. Patients increasingly seek practitioners who can address their unique health challenges comprehensively and holistically.</span></span></li><li style="font-weight: 400;" aria-level="1"><span style="font-family: Thabit, sans-serif;"><b><i>We can build long-term patient relationships.</i></b><span style="font-weight: 400;"> Diabetes is a chronic condition that requires ongoing management and monitoring. By actively engaging in diabetes care, we can establish long-term relationships with our patients. This continuity of care fosters patient loyalty and ensures consistent revenue for the practice over time.</span></span></li><li style="font-weight: 400;" aria-level="1"><span style="font-family: Thabit, sans-serif;"><b><i>We can participate in a collaborative care model.</i></b><span style="font-weight: 400;"> Diabetes care often involves collaboration with other healthcare professionals, such as primary care physicians, endocrinologists and dietitians. A practice that actively participates in a multidisciplinary approach to diabetes management becomes an integral part of a patient’s healthcare journey. This collaborative model strengthens professional networks and referrals.</span></span></li><li style="font-weight: 400;" aria-level="1"><span style="font-family: Thabit, sans-serif;"><b><i>We can receive a fair reimbursement using efficient, well-developed diagnostics.</i></b><span style="font-weight: 400;"> There are mechanisms for compensation for diabetes care delivery. There is a national commitment to the prevention of diabetes complications, including diabetic retinopathy, which no doubt contributes to the less restrictive coding for necessary tests. For example, when a patient presents with diabetes and uncontrolled HA1C, we can and should go beyond fundus photography and visual acuity as measures of structure and function in visually symptomatic patients. The utility of OCT in such cases cannot be understated given that we can now examine the retinal microvasculature like never before. Likewise, the electroretinography (ERG) provides an objective measure of function that is far more useful in practice than subjective visual acuity assessments. For example, a technician can collect an objective DR Score within minutes</span><span style="font-weight: 400;">5 </span><span style="font-weight: 400;">with a handheld </span><a href="https://lkc.com/products/reteval/" target="_blank" rel="noopener"><b>RET</b><b><i>eval</i></b></a><span style="font-weight: 400;">, and color-coded results on a number line provide actionable data and a reimbursement of ~ $128 with the availability of more than 560 diagnostic codes.</span></span></li></ol><p><span style="font-weight: 400; font-family: Thabit, sans-serif;">The financial rewards and positive impact on public health make diabetes care a cornerstone for the success and sustainability of optometrists who are willing to embrace this transformative approach.</span></p><h4><span style="font-family: Thabit, sans-serif;"><b>Results of Our Focus on Diabetes Care</b></span></h4><p><span style="font-weight: 400; font-family: Thabit, sans-serif;">The number of people we see with diabetes continues to increase in our practice, and has grown by 3 percent per year over the last five years as a result of the overall increase in the number of people with diabetes, the aging population in our rural location and the overall optometry and ophthalmology provider shortage. We project this number will continue at this pace for the next 10 years!</span></p><p><span style="font-weight: 400; font-family: Thabit, sans-serif;">We are providing more diagnostic and supportive care for those living with diabetes than ever before because many present with the comorbidities of hypertension, glaucoma, cataracts and other ocular and systemic conditions.</span></p><p><span style="font-weight: 400; font-family: Thabit, sans-serif;">The diagnostic testing to best care for these complex patients increases the revenue per visit by as much as 50 percent. There are operational costs to taking care of more complicated patients, but the overall financial yield is still in the positive compared to routine care visits.</span></p><p><span style="font-weight: 400; font-family: Thabit, sans-serif;">As primary eyecare providers, we play a pivotal role in enhancing community health, particularly as it relates to elevating the standards of care in diabetic eye disease. There is absolutely not a single doubt that those patterns of marketing strategies have yielded tremendous results in Optometry. In fact, those are marketing practices that have been developed by practice owners who are demonstrating excellent entrepreneurial behavior time after time and are remarkably paving the way to other practitioners to employ and improve those strategies. In this article, I try to delve first into listing what is commercial marketing that allows myopia management practitioners to market their practices and then define social marketing strategies for myopia management trying to demonstrate how commercial marketing fits into social marketing the myopia disease.</p><p>Social media and other content-sharing platforms offer countless chances for online interaction these days. Our daily lives are largely reliant on the internet, which provides the best means of obtaining news and information. As a matter of fact, eight out of ten individuals state that they get most of their news from digital devices, such as social media apps or online news sources (Shearer, 2021). Health experts and eye care professionals in particular can use social marketing and other health promotion methods at different levels in their marketing strategy.</p><p>Social marketing, on the other side, employs marketing techniques typically used in commerce to influence behavior regarding social issues. Crafting a social marketing strategy enables a more targeted and effective approach in communicating with specific audiences, leading to improved outcomes. It’s essential to differentiate social marketing from social media, as the latter, including platforms like Facebook and Twitter, serves as tools within the broader framework of social marketing plans to achieve specific objectives.</p><p>Optometry practices are implementing different commercial marketing strategies and social media strategies to effectively raise awareness, attract patients, and establish themselves as a trusted provider in the community. Here are a few of them:</p><ol><li>Patient Education during Exams: Utilize exam time to educate patients and parents about myopia management, emphasizing the risks associated with untreated myopia and the benefits of early intervention (Kazmi, 2022).</li><li>E-Blast Marketing: Target e-blasts to the patient base, highlighting new treatment options or services. Segment lists to focus on patients who may benefit from myopia management (Kazmi, 2022).</li><li>Blog & Vlog Series: Establish a regular blog and vlog series to provide in-depth information about myopia, its prevalence, associated risks, and available treatment options. Consistent content creation builds credibility and engages the community (Kazmi, 2022).</li><li>Staff Training and Education: Educate office staff about myopia management services and treatments. Provide materials and resources to facilitate patient education and engagement. Ensure staff members can effectively communicate the benefits of myopia management (Aleman-Moheeputh,2021).</li><li>Word-of-Mouth Marketing: Encourage satisfied patients to spread the word about the practice’s myopia management services. Leverage patient testimonials and success stories to build trust and credibility (Aleman-Moheeputh,2021).</li><li>Networking with Local Optometrists: Foster collaborative relationships with other eye care professionals to exchange referrals and support. Participate in local events and professional organizations to expand the practice’s network (Aleman-Moheeputh,2021).</li><li>Online Presence and Social Media: Maintain an updated Google business listing and actively engage on social media platforms. Share informative content, success stories, and practice updates to stay top-of-mind among patients and colleagues (Aleman-Moheeputh,2021).</li></ol><p>Andreasen (1995), in the preface to “Marketing Social Change,” defines social marketing as applying commercial strategies to address various socially significant behaviors, like drug use and smoking. This approach, drawing from established concepts and techniques, has substantial potential to address significant societal issues if effectively utilized. The “proven techniques” encompass methods from behavioral theory, persuasion psychology, and marketing science. These techniques involve leveraging behavioral theory to influence health-related behaviors, evaluating audience receptivity to messages, and strategically marketing messages to drive behavior change using the “four Ps” of marketing (Kotler & Lee, 2008):</p><p style="padding-left: 40px;">1- Behavior Change: Initiate campaigns targeting behavior change towards myopia prevention, emphasizing the importance of outdoor activities and reducing screen time for eye health.</p><p style="padding-left: 40px;">2- Customer Research: Conduct comprehensive research to understand myopia prevalence, risk factors, and prevailing attitudes towards eye health within target populations, enabling tailored intervention strategies.</p><p style="padding-left: 40px;">3- Audience Segmentation: By segmenting their target audience based on factors such as age, lifestyle, and eye health needs, optometrists can tailor their marketing messages to resonate with specific groups. For example, they can develop messages targeted at parents concerned about their children’s eye, the need for early eye exam, emmetropes prevention ways, pre-myopic management tools, myopia, and high myopia management methods.</p><p style="padding-left: 40px;">4- Marketing Mix:</p><p style="padding-left: 80px;">· Product: Position outdoor activities as desirable ‘products’ associated with myopia prevention, offering benefits such as improved overall health and reduced risk of vision impairment.</p><p style="padding-left: 80px;">· Price: Address perceived barriers such as inconvenience or lack of awareness by offering incentives like free outdoor activity guides or discounts on protective eyewear.</p><p style="padding-left: 80px;">· Place: Promote myopia prevention interventions in settings where the target audience spends significant time, such as schools, workplaces, and online platforms.</p><p style="padding-left: 80px;">· Promotion: Utilize various channels including social media, educational campaigns, and partnerships with influencers to amplify messages and engage the audience effectively.</p><p style="padding-left: 40px;">5- Communication Channels: Optometrists can utilize a variety of communication channels to reach their target audience, including social media, community events, and local advertising. By engaging with potential patients through these channels, they can raise awareness about the importance of regular eye exams and promote their services effectively.</p><p style="padding-left: 40px;">6- Tailored Messages: Optometrists can create personalized messages that address the unique needs and concerns of their target audience. For example, they can highlight the benefits of specific eye care services such as myopia management at the juvenile onset.</p><p style="padding-left: 40px;">7- Brand Building: By developing a strong brand identity that emphasizes expertise, trustworthiness, and patient-centered care, optometrists can differentiate themselves from competitors and attract more patients. Consistent branding across all marketing channels helps reinforce their reputation and build long-term relationships with patients.</p><p style="padding-left: 40px;">8- Reinforcement of Public Health Messages: Optometrists can reinforce public health messages related to eye care, such as the importance of regular screenings and good visual habits like less screen time. Additionally, they can enhance outreach by offering an additional avenue for communication to engage the target audience. Recent research suggests that contoured prism may alleviate symptoms such as asthenopia, headaches and neck pain among those with even small eye misalignments. Contoured prism varies the amount of prism across the lens to address the viewer’s prismatic needs at different distances.</p><p>As ECPs, we can better enhance patients’ binocular vision functioning as well as alleviate associated discomfort when we know when and how to leverage prism with patients. Let’s take a deeper look at this fundamental principle of ophthalmic optics and see how we can harness its power.</p><h4><strong>Binocular Sensory Perception</strong></h4><p>Binocular vision affords a larger field of view, compensates for the optic disc scotoma and enhances depth perception as compared to vision with a single eye. These benefits rely on the interpupillary distance. Since the two eyes are separated horizontally on the face, the light reflecting off objects and entering the eye form slightly different images, a phenomenon referred to as retinal disparity. Yet, those with binocular vision see a single image. This conundrum has fascinated thinkers throughout the ages.</p><p>Macular perception is the most basic level of sensory status in binocular vision. The visual axis of each eye connects the object, through the center of rotation of the eye, to the fovea. Fusion, the next grade of sensory status, occurs not within the eye but further along the visual pathways and results in the perception of a single image. In stereopsis, the highest grade of binocular sensory perception, the neural circuitry in the visual pathways uses complex computations to detect the location of objects in three-dimensional space.</p><p>Binocular sensory perception requires the eyes to converge or diverge so that the visual axes continue to connect objects at different distances to the fovea of each eye. This is accomplished by the extraocular muscles. For convergence, the medial rectus muscles contract while the lateral rectus muscles relax. In divergence, the lateral rectus muscles contract while the medial rectus muscles relax. Vergence responses must coordinate with accommodation to keep the image clear. Phorias and tropias involve misalignment of the visual axes. A manifest tropia is always apparent. Intermittent tropias are only apparent some of the time. Phorias last a very short time. Researchers and clinicians have developed scales to rate eye turn duration from pure phoria all the way up to manifest tropia, according to Clinical Management of Binocular Vision by Mitchell Scheiman and Bruce Wick.</p><p>Clinicians also use prisms to determine the magnitude of a phoria or tropia. The general idea is to use high prism to dissociate the images between the eyes. In other words, the clinician purposefully induces diplopia. Dissociating the images allows the clinician to observe the behavior of the eyes independently. Dr. Sheiman and Dr. Wick describe the von Graefe technique that uses this approach. The left eye views an optotype (e.g., letter on a Snellen chart) through a horizontal prism and the right eye views through a vertical prism. The horizontal prism is approximately 10^ to 12^BI and the vertical prism is about 6^BD. To measure the horizontal deviation, the clinician adjusts the magnitude of the horizontal prism until the patient says that the top and bottom images of the optotype align. This measurement technique is often performed at distance and near because the magnitude of the phoria is typically different at these distances. Most people tend to have a small exophoria. The expected mean distance horizontal deviation for adults is 1 prism diopter exophoria with a standard deviation of +/-1 prism diopter. So, approximately 68 percent of the population has a horizontal deviation of 0 prism diopters (orthophoria) to 2 prism diopters exophoria based on the statistical normal distribution. For near, the expected mean horizontal deviation for adults is 3 prism diopters exophoria with a standard deviation of +/- 3 prism diopters. So, approximately 68 percent of adults have 0 prism diopters (orthophoria) to 6 prism diopters exophoria at near.</p><p>When fixating on an object, two forces are in play. One force moves the visual axes according to the phoria; the other force moves the visual axes toward the object through the vergence response. Fixation disparity is the difference between the visual target vergence angle (the angle created by connecting the eyes to the target) and the viewer’s vergence angle under binocular viewing. In other words, measures of fixation disparity indicate the viewer’s ability to compensate for a phoria.</p><h5>Binocular Function, Pain and Discomfort</h5><p>What happens if a patient’s phoria is within the typical range for distance but outside the range at near or vice versa? In this case, the patient’s eyes are converging too much (convergence excess) or too little (convergence insufficiency), or diverging too much (divergence excess) or too little (divergence insufficiency). These conditions are associated with symptoms such as asthenopia, headaches, intermittent blur or diplopia, burning and tearing. Researchers are currently investigating whether these symptoms also arise from smaller, more typical phorias. The trigeminal nerve is a cranial nerve that transmits sensory information related to temperature, touch and pain from the face, including the eyes, to the trigeminal nucleus, which projects to other parts of the nervous system. One theory under investigation associates small eye misalignments with a broad cluster of symptoms referred to as trigeminal dysphoria. More research is needed to confirm this theory and investigate the mechanisms involved.</p><h5>Uniform Wedge Prisms</h5><p>Using uniform prism is one strategy for addressing the symptoms associated with binocular vision disorders. A prism bends light toward the base and displaces the image toward the apex. When the image appears in a different place, the eye’s visual axis must point in a different direction to maintain fixation on the object. Most simply, we can insert a plano wedge prism into a frame for a patient without a refractive error. In practice, wedge prisms are rarely used for daily use. A lens with uniform base up prism would move the image down toward the apex so the eye would turn down to align with the new location of the image. Uniform base down prism has the opposite effect—the image is displaced up so the eye turns up. Uniform base in prism displaces the image temporally so that the eye turns out and base out prism displaces the image nasally so that the eye turns in. In practice, plano lenses with uniform prism typically have a positive base curve.</p><h5>Ophthalmic Lenses and Prism</h5><p>As we know, non-plano ophthalmic lenses are designed to converge light to correct for hyperopia and diverge light to correct for myopia. To do so, plus lenses are thickest at the optical center and thinner around the edges. In essence, the edges of a plus lens are an apex, and the optical center is a base of a concentric prism. The opposite is true of a minus lens: the edges are a base, and the optical center is the apex. In contrast to the plano prisms described above, the plus and minus lenses also curve at different rates on the front and back surfaces, so the amount of prism changes based on the dioptric power. For spherocylindrical lenses, since the dioptric power changes based on the meridian (as found using the oblique meridian formula), the amount of prism also changes based on the meridian. Prentice’s Rule describes this relationship mathematically. The amount of prism in prism diopters is the product of the dioptric power along the given meridian and the distance from the optical center in centimeters.</p><h5>Prescribed Prism</h5><p>Patients often benefit from prism while still having a refractive error. The lens must do double duty: displace the image and correct the refractive error. One way to accomplish both tasks is to decenter the optical center of a spherical or spherocylindrical lens. Then the patient’s visual axis at primary gaze passes through a point on the lens with the desired amount of prismatic effect. Another option is to surface the lens so that it already combines the spherical or spherocylindrical curvature with uniform prism.</p><p>We can use prism to move an image of an object, but in which direction should we move the image? We can take two approaches: Move the image toward the patient’s existing visual axis, or move the patient’s visual axis toward the object. Moving the image toward the patient’s existing visual axis is relieving or therapeutic prism. The prism relieves the need for the extraocular musculature from constantly attempting to fixate an object with both eyes. This can make fusion easier. In this case, the base is placed on the opposite side from the deviation. Moving the visual axis toward the object is adverse prism. To do so, the base is placed in the direction of the deviation. The prism challenges the extraocular musculature to build up the strength and skill to fixate on the object.</p><h5>New Measurement Techniques</h5><p>In October 2023, Neurolens, a company specializing in contoured prism technology, introduced a new virtual reality-based device that measures binocular alignment. Like previous Neurolens measurement devices, the N3 measures phorias at distance and near, fixation disparity and accommodative convergence response, according to a Neurolens blog post. Similar to the von Graefe technique, the N3 uses a dissociative test with non-fusible targets to measure eye misalignment. N3 also measures fixation disparity and the accommodative convergence response.</p><p>The N3 guides the patient through the measurement process, which can be conducted in the pretesting area or exam room. After the assessment, while the patient is still immersed in virtual reality, the device demonstrates any misalignment findings to the patient and asks whether the patient would like to discuss the findings with the optometrist.</p><p>Tausha Barton, OD, a consultant for Neurolens, began using the N3 in October 2023. Among her first patients were two brothers, ages 9 and 11. After the children completed the testing, the 9-year-old reported “Yeah, it was easy… it said I have an eye misalignment and need special glasses to fix it.” When Dr. Barton looked at the results, she noted that the 19-year-old had exophoria at distance and esophoria at near, an uncommon misalignment. Based on these findings, Dr. Barton asked the mother about the younger boy’s reading performance at school. The screening questionnaire before testing had not revealed any symptoms. Following up on the N3 results exposed a more detailed picture. When asked how the youngest boy was doing in reading, the mother reported that his reading performance was below expectations and that he avoided reading. Dr. Barton suggested that the reading difficulties may be due to eye misalignment at near rather than a learning difference: “If a child’s eyes are uncomfortable, they may avoid things that make them uncomfortable…They don’t have the vocabulary or knowledge that what they are experiencing with words moving on the page or splitting apart is not normal.” One of the children also volunteered that when he is watching TV, the image splits into two. When he blinks a few times, the image returns to a single image. Neurolenses may enhance the children’s quality of life by improving their binocular vision. Dr. Barton prescribed Neurolenses as a solution to improve the boys’ binocular vision.</p><p>Ophthalmic optics researchers, suppliers and eyecare professionals continue to master the technology of prisms to move images to their advantage. Technology such as the N3 measurement device and contoured lenses bring precision to binocular function assessment and ophthalmic lens design. As experts in ophthalmic lenses, opticians have an opportunity to be leaders in this process.</p> </div> </div> <div class="elementor-element elementor-element-79e6c86 elementor-widget elementor-widget-text-editor" data-id="79e6c86" data-element_type="widget" data-widget_type="text-editor.default"> <div class="elementor-widget-container"> <h4>Contoured Prism</h4><p>Opticians specialize in manufacturing and fitting lenses to optimize patients’ visual experience in natural settings. As such, more optical labs such as Cherry Optical Lab are beginning to manufacture contoured prism lenses through freeform digital technology. Dispensing opticians ensure that the eyes and contoured prism lenses work together as a system. Opticians also continue the education begun in the exam room. Patients may not have been exposed to information about binocular vision and eye misalignment in previous settings. Opticians can help patients distinguish between visual acuity and binocular function. Patients may expect lenses with contoured prism to help images to appear clearer so opticians can highlight that the purpose of contoured prism is to counteract eye misalignment and improve symptoms such as headaches and eye strain.</p><p>The Neurolens measuring devices, including the most recent N3, provide the data needed to produce lenses with contoured prism. Freeform technology precisely contours the lens so that the amount of prism varies to account for eye misalignment differences at distance and near. Since most people have less misalignment at distance than at near, the amount of prism in a contoured lens typically increases gradually from distance to near. Often this is compared to progressive lenses: in progressive lenses, the dioptric power gradually increases from distance to near; in contoured prism, the amount of prism gradually increases from distance to near. Presbyopes may benefit from both progressive lenses and lenses with contoured prism. The mother’s headaches occurred five to seven days per week. She had tried a variety of treatments including chiropractic, massage, botox injections and antidepressants. The mother was fitted with contoured lenses. The mother reported to Dr. Barton that the night after wearing the Neurolenses was the first night she had gone to bed without neck pain in 10 years.</p><p>Neurolens completed a clinical trial of the effect of contoured prism on headache reduction in May 2022. Publications are not yet available on the National Library of Medicine website. In today’s DIY climate it can be a mistake to think that the budget should be based on where you can cut corners.</p><p>Make sure your budget includes funds for building a demographically correct design with the right lighting and with commercial grade furniture <em>and</em> also provides for immediate and future growth requirements. Including these things in your optical budget is essential to increase your capture ratio and overall sales.</p><p>Additional Tip: Make sure you negotiate enough favorable terms if you’re leasing. The result is a lasting, positive patient experience and loyalty to the practice.</p><p><strong>Understanding the Modern Consumer</strong></p><p>During the past decade, the world of eyecare changed dramatically, not just for practitioners but for their patients. Consumers have a host of options for purchasing eyeglasses and sunglasses. They will do business with a provider who makes them feel comfortable while satisfying their eyecare needs and wants at prices they personally can manage. It is the time of the more sophisticated consumer in the eyewear field. Buy Now, Pay Later is a short-term financing tool that offers patients options to make payments over a specified period. Patients are empowered to purchase what they want when they want it without compromise.</p><p>The eyewear consumer is knowledgeable, and before they walk through the door to make an instore purchase, they have already compared products and prices across multiple channels. They enter the practice with solid notions of what they want, and most importantly, they want it now, if not yesterday. When purchasing eyewear, they look at the clock, not the calendar. Time is the new currency for patients.</p><p>That is why practice owners keen on providing the best service find ways to make the most use of every minute of the day. One way: simplifying the checkout process, maximizing time for both patient and practice staff in two ways.</p><p>For the ECP change brings discomfort, which slows the transition to new ways of doing business. According to Laura Kreofsky, VP, strategy, at Pivot Point Consulting, “The retail health care revolution poses an existential challenge to traditional providers, many of which have been slow both to embrace digital technologies and acknowledge the growing expectations of health care consumers for choice, affordability and personalization.” All is not lost, as Kreofsky explains: “By developing consumer-focused strategies that include the deployment of virtual care services and user-friendly technologies and ongoing support, traditional health care providers can protect their revenue streams while strengthening patient loyalty. The good news for providers is they also can deploy technologies and develop strategies to retain patients by meeting their needs for a superior experience.”</p><p>It is a bit ironic that one of the “newest” ways of satisfying patients’ expectations comes with the rebirth of a once-popular retail strategy: Buy Now, Pay Later (BNPL), modified of course to meet the demands of the 21st century patient.</p><p>Too often, practitioners, as with many business providers, talk about a purchase driven by common sense. The patient should ask, “What are the best eyecare products for my needs?” And there is the dreaded question for all retailers from consumers: “Are you giving me the lowest price for this product?” Study after study has shown that reason plays a small role in most purchasing decisions.</p><p>Emotion, more than logic, drives purchasing decisions. It isn’t about which product at what price. It’s the experience the consumer feels. By offering personalized and human experiences, the practice’s staff reaches patients on an emotional level. A well-tuned staff generates an atmosphere of trust where the patient feels comfortable asking questions and expressing concerns. The patient feels a connection. This connection and trust minimize the importance of price and offering BNPL frees the patient to fall in love with a beautiful pair of eyewear and sunglasses without worrying about affording them.</p><p>There’s an additional benefit to this technique, one you never witness, but it will help your practice grow. When patients receive personal interaction and develop a bond with the practice, they happily tell their family and friends where they got their new eyewear. More importantly, they tell their acquaintances about how simple and comfortable the process was.</p><p>Knowing revenue ultimately reflects customer satisfaction, ECPs rank customer satisfaction as the most critical business metric for the second year in a row. Capture rates come in second, with 43 percent of respondents in a Sunbit research survey saying they are currently concerned or very concerned about capture rates. The top two metrics are related: Patients are more likely to be satisfied if they walk out with the eyewear they need and are able to see, but cost is the number one barrier to capture rates.</p><p>Many sectors of the U.S. economy have used this precise process to enable consumers to buy the product they need quickly. Think of automobiles. How many Americans can shell out $35,000 for a new car? Most cannot. That is why dealerships have gone into various programs that make the acquisition more financially comfortable for the consumer. You might say, “Well, eyewear doesn’t cost nearly as much.” True. Think then of appliances. People spend thousands on washers, dryers, refrigerators and ovens. Most have not saved for the expenditure, but they need them. So, stores arrange programs that lower the impact of the price, enabling the consumer to buy the product they need and want. Besides, isn’t eyewear more important than a car or an appliance, both of which are worthless if you cannot see well enough to use them?</p><p>Essentially our eyewear patients are looking for the same comfortable financing flexibility offered by other sectors of the economy. In 2020, 44 percent of consumers said a BNPL service is somewhat or very important in determining how much they spend. Consumers want fast and easy access to financing. They want you to listen, then have a mutually beneficial dialogue. Consumers want a genuine transparent purchase process that shares their values.</p><p><strong>How Does BNPL Build Business?</strong></p><p>Your patient fell in love with a luxury frame and needs glare and blue light protection in a premium lens, but the cost is outside of her budget. How can you avoid that patient’s disappointment while helping her achieve the best vision with a style she’ll want to wear? BNPL is a short-term financing tool that offers consumers options to make payments over a specified time. BNPL helps patients find value in premium products and multiple pairs. The buy-now-and-pay-later option reframes how a patient sees the cost of the transaction. It frees them to go with the preferred option, making closing the sale easier for the optician and patient. Let’s see why.</p><p>While eyecare has played an integral role in the health care field for more than two centuries, its services and products are ignored, for example, with health insurance plans. According to Versant Health, vision care usually is one of the least accessible health care benefits with employee subsidized insurance. It is often presented as an ancillary service in health care plans. The lack of a strong third-party payer in the eyecare field adds to the friction of purchasing. Consumers today are keenly aware of the costs associated with vision care and will seek ways to mitigate them. Nearly every optician has encountered the patient who says, “I only want what is covered.” But the truth is, no one really wants only what is covered by their insurance. Most of the time, they are trying to say that they want a nice pair of glasses, but their budgets are not large enough to cover the costs associated with their preference.</p><p>One way of handling these pressure points is financing options such as Sunbit. These BNPL programs enable managed care patients to get the eyewear they want without being held back by the limits of their managed care plan. Therefore, BNPL provides ease of purchasing, essential to a positive customer experience.</p><p>According to a Sunbit ECP Survey, January 2022, the average out-of-pocket expense for eyewear was $343. By signing up with a program like Sunbit’s, patients gain options they may find manageable. Patients are empowered to purchase what they want when they want it without compromise. Sunbit removes barriers to purchasing, including the multiple pairs everyone needs.</p><p>According to The Vision Council Covid-19 consumer market insights webinar published in March 2020, two out of three consumers surveyed said they would put off in-person visits for eyecare until the pandemic had passed, and those planning to make online purchases of eyewear more than doubled. Versant Health noted that 88 percent of consumers now prefer online shopping, and that the majority of online eyewear purchases in 2020 were made by Millennials (ages 24 to 39). Online purchases create opportunities for consumers but increase competition for practices, especially with Millennials accustomed to purchasing everything from underwear to automobiles online. Simultaneously, more than 50 percent of consumers used BNPL in 2021, many of them exposed to it online. Consciously or unconsciously, these consumers expect the same option from their in-person optical providers. According to some experts, BNPL plans even the playing field for the independent optical practice with e-commerce competitors. In addition, they can offer the personal service patients expect that cannot be obtained online.</p><p>Consider that most consumers, particularly Millennials, are wary of accruing credit card debt. Patients see “the light at the end of the tunnel” when they know exactly how much they will pay and how long it will take. Then add professional attention and patient education to provide a uniquely personal experience. Sunbit is an added amenity and convenience to the eyewear purchase experience, increasing patient comfort levels and making informed decisions. A better consumer experience results in patient loyalty, and loyal, happy patients tell friends and family. Even the National Eye Institute suggests that people ask friends, family, coworkers or neighbors if they can recommend a vision care provider if they don’t have one, making a good word of mouth reputation important.</p><p>At the same time, Internet sales have changed expectations. Online, they are three or four clicks from checking out. Of course, they do not expect the same in your practice, but their clocks are ticking faster than they were only 10 years ago. Streamlining checkout speeds up the process. Additionally, it saves staff time. With the additional time, staff can invest more time into providing the personal attention of customer service. Staff guide patients through the fast, easy application process rather than having them sign up on their own. The financing process is integrated into the practice’s point-of-sale system. You can offer patients flexible credit at fair and reasonable rates, with a quick application process and transparent terms. Your patients choose the payment option they want without concern for long-term debt. And if you have an in-office lab, with Sunbit’s 30-second approval process, your patients can take home the excellent pair of glasses they want with peace of mind.</p><p><strong>How Sunbit Benefits the Independent Practice</strong></p><p>Sunbit research shows that optical offices want to make the patient experience better. They want to achieve high customer satisfaction rates and high capture rates. The research also reveals how ECPs can tackle both of those goals simultaneously. See what they’ve found out and how it can impact your business.</p><p>Consider these findings: Almost 70 percent of people who wear glasses, contacts or both spend $100 or more. Nearly half spend $200 or more. The average cost of frames and lenses in the U.S. is $343. Members of Consumer Reports have likewise indicated that cost is one of their chief considerations when choosing an eyewear retailer.</p><p>And there’s more: 93 percent of ECPs are open to offering financing of some kind. That said, many ECPs are not aware of the available options. The highest awareness is 39 percent for medical credit cards, 19 percent for practice backed payment plans and 17 percent for BNPL. Of all the financing options, BNPL has the most potential to boost customer satisfaction rates AND capture rates. Customers want payment plans. If given a payment plan option, 78 percent of people would upgrade their lenses or purchase more eyewear. Sunbit can be used to pay for the exam as well.</p><p>On the journey from the patient in the exam room to the consumer in the optical dispensary, practices need to leverage technology. Sunbit was founded in 2015, offering new technology to help people by making financing more accessible for essential services. Sunbit isn’t a credit card, which many patients don’t want. They offer access to finite installment loans for which a 30-second application process results in 90 percent approval. For example, it is estimated that by 2023, 23 percent of optical sales will be through e-commerce. As patients return to physical stores after the increase in online purchasing over the past two years, they expect the same options e-commerce offered, but with the added benefits of personal connection and trust. They want (maybe demand is a better description) the process to be easy, emphasizing service even more than price. Practices can distinguish themselves from e-commerce by offering patients dignity, respect and care, including easy financing. Small and medium-sized businesses can compete with online retailers and offer the same BNPL as larger companies by offering Sunbit. The practice is paid right away. The onboarding process is initiated in the office, so patients don’t have to look for it independently. The practice receives Sunbit utilization information in regular financing reports so they can see the difference it’s making.</p><p>Sometimes staff assumes they know the limits of a patient’s budget and decide how much they are willing to spend. Sunbit conquers that issue when staff understands that the patients choose their payment options for the eyewear and services they really want. If you’re interested in offering a BNPL option, look for a solution that offers:</p><ul><li>A 100 percent digital application process.</li><li>A quick answer without a hard credit check.</li><li>High approval rates—at least 90 percent.</li><li>Simple interest loans with fair rates.</li><li>No hidden fees, no late fees.</li><li>Quick payment in full to the ECP.</li></ul><p>Sunbit checks all of those boxes.</p><p>ECPs have already seen dramatic success with BNPL technology solutions like Sunbit. Having the option to spread payments over time helps redirect the conversation with patients to focus on the eyewear they want. Staff can be comfortable and establish a rapport with the patient. A good listener will convey empathy and understanding of the patient’s needs. Payment options show that you are flexible and will find creative solutions to fill those needs. Identify any objections. Clarify any misunderstanding of the product and patient needs. Explain and demonstrate the features, benefits and advantages of the product. Stress value over price, and by making the purchasing process more straightforward, patients will embrace value.</p><p><strong>Conclusion</strong></p><p>Life as a practice owner can be hectic with juggling patients, maintaining operational initiatives, and an office full of employees. Making even one thing more straightforward and streamlined would be a welcome benefit.
